The Apostles’ House is seeking a dynamic and compassionate individual to join the Parent Aide team. Our Parent Aide Case Managers are professionally trained individuals who provide in-home support and encouragement, model appropriate parenting techniques, focus on the good qualities of the parents, and work with the family to address any challenges or barriers families are facing by referring them to community agencies when necessary.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
· Provide home visitation services to a caseload of 10 to 15 families, according to the Parent Aide Program model.
· Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
· Develop service plans, review case progress and determine case closure
· Help clients achieve wellness and autonomy
· Record cases information, complete accurately all necessary forms, and produce statistical reports
· Facilitate multiple care aspects (case coordination, information sharing, etc)
· Help clients make informed decisions by acting as their advocate regarding their benefits status and housing options
· Develop effective working relations and cooperate with the administrative team throughout the entire case management process
· Promote quality interventions and outcomes
· Assess and address motivational and psychosocial issues
· Adhere to professional standards as outlined by protocols, rules, and regulations
JOB QUALIFICATIONS:
· Proven working experience in case management, including as a nurse, medical, mental health care manager, or related field
· Excellent knowledge of case management principles, healthcare management
· Previous experience with psychological aspects of care
· Effective communication skills
· Excellent organizational and time management skills
· Familiarity with professional and technical emerging knowledge
· Problem-solving skills and ability to multi-task
· Compassionate with teamwork skills
· BS degree in social work, psychology, or human services preferred
REQUIREMENTS:
. Valid driver’s license and reliable vehicle
. Experience working with at-risk families
. Speak and understand Spanish

SALARY RANGE:
$21.00 – $23.00 per hour
